Submit a Governor's Points of Light Nomination

Submitting Nominations

To submit a nomination application please select the appropriate nomination category using the links above. Once you begin the nomination form you will not be able to save and return to the form later. Please be prepared to complete the entire nomination at one time. View the instructions and information you will need to complete the form below.

The online nomination form requires:

o Category of the nomination

o Nominator (person completing the form) and nominee contact information

o Full name

o Address

o Phone number

o Email

o Contact information for three references who are familiar with the nominee's service.

o References cannot be the nominator

o References cannot be a member of the nominee’s immediate family (i.e. spouse, significant other, mother, father, sister, brother, or child)

o Narrative Sections on Excellence in Volunteerism, Commitment, Impact, Use of Resources.  Each of these sections has a total of 10 possible points.  Each section has a Minimum and Maximum Character limit.

o Excellence in Volunteerism/Service:  Please describe how the nominee demonstrates accomplishments that are exemplary and exceed average volunteer performance. (10 possible points, 400 minimum and 2,500 maximum character limit)

o Commitment:  Please describe how the nominee demonstrates a commitment to advancing a community or cause, that is exemplary and exceeds average volunteer performance. (10 possible points, 400 minimum and 2,500 maximum character limit)

o Impact:  Please describe how the nominee's service achieves results that are evident and advance a cause or improve a community. (10 possible points, 400 minimum and 2,500 maximum character limit)

o Use of Resources:  Please describe how the nominee demonstrates exemplary use of people, resources and opportunities. (10 possible points, 400 minimum and 2,500 maximum character limit)

o The Lifetime Achievement has 2 additional narrative sections.  (10 possible points each, 400 minimum and 2,500 maximum character limit)

o Longevity:  Please describe how the nominee has demonstrated an inherent ethic of service and desire to improve their community through lifelong volunteerism. 

o Inspiration:  Please describe how the nominee demonstrates an exemplary ability to inspire others to serve and make a difference in their community.

We suggest nominators use a word document to prepare and record answers to fields, so if something does happen you will have a saved copy of the responses to each field.

Nominations must be submitted by 11:59 pm on Friday, November 30, 2019.
